What you will do

• Conduct virtual wellness visits, health assessments, and cognitive screenings.
• Collaborate with clinical in-home teams to evaluate vital signs and arrange blood draws.
• Provide comprehensive care management including medication reviews and specialist referrals.
• Document patient findings and care plans using secure health technology.
• Educate patients on preventive care and community resources.
• Manage daily schedules and patient caseloads using digital tools.

Requirements

• Board Certified as a Family Nurse Practitioner (FNP).
• Active Family Nurse Practitioner License in Illinois.
• Professionalism and ability to handle high-quality clinical standards independently.
• Technologically savvy with experience in digital charting and secure messaging.
• Strong written and verbal communication skills for patient interactions.
• Ability to work autonomously in a remote environment.

Nice to have

• Additional state licenses (GA, NC, SC, FL, and/or MS).
• Experience in telemedicine or mobile health service models.
