What you will do

• Facilitate patient enrollment into Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) programs.
• Conduct daily calls and text messages to document patient health status and maintain engagement.
• Collect and document detailed patient health information, including histories and new symptoms.
• Support patients by providing health pointers such as stretches and meditation videos.
• Maintain strict confidentiality and adherence to HIPAA regulations and company policies.
• Participate in ongoing company training and team check-ins.

Requirements

• Certification as an LPN.
• 3+ years of experience working in an office or remote environment.
• Proficiency with Zoom, Slack, and online enrollment platforms.
• Strong organizational skills and the ability to manage multiple tasks efficiently.
• Excellent verbal and written communication skills with a focus on empathy.
• Ability to work independently from a quiet space with a reliable internet connection.
