What you will do

• Review caseloads and prepare technology for patient services.
• Navigate schedules and partner with clinical in-home team members (Sprinters).
• Make connections with a diverse range of patients to collect relevant information and perform various services.
• Conduct health risk assessments, medication reviews, and cognitive screenings.
• Work directly with Sprinters to evaluate vital signs and arrange blood draws.
• Provide comprehensive care coordination and management, including preventive care interventions and referrals.

Requirements

• Board Certified as a Family Nurse Practitioner in GA, NC, SC, FL, and/or MS.
• Active Family Nurse Practitioner License in California.
• Consistently exhibits professionalism, integrity, accountability, and compassion.
• Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking.
• Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination.
• Strong written and verbal communication skills.
