What you will do

• Review caseload and prepare technology for patient services.
• Navigate the daily schedule using modern technology, partnering with clinical in-home team members (Sprinters).
• Make connections with a diverse range of patients, collecting relevant information and performing various services.
• Conduct health risk assessments, medication reviews, and cognitive screenings.
• Work with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks.
• Provide comprehensive care coordination and management, including preventive care interventions and referrals.

Requirements

• Board Certified as a Family Nurse Practitioner in KY, MI, OH, and/or IL.
• Consistently exhibit professionalism, integrity, accountability, confidentiality, care, and compassion.
• Willingness to work in a revolutionary environment requiring problem-solving and out-of-the-box thinking.
• Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps.
• Strong written and verbal communication skills.
• Ability to work independently or in a team environment.
• Pass national background check and valid clinical license search.
