What you will do

• Review caseload and prepare technology for patient services.
• Navigate daily schedule and partner with clinical in-home team members (Sprinters).
• Make connections with patients, collect information, and perform services.
• Conduct health risk assessments, medication reviews, and cognitive screenings.
• Work with Sprinters to evaluate vital signs and arrange blood draws.
• Provide care coordination, including preventive interventions and referrals.

Requirements

• Board Certified as a Family Nurse Practitioner in GA, NC, SC, FL, and/or MS.
• Active Family Nurse Practitioner License in California.
• Consistently exhibits professionalism, integrity, and compassion.
