What you will do

  • Coordinate mental and physical health care for assigned patients in collaboration with providers.
  • Screen and assess patients for mental health and substance use disorders.
  • Deliver evidence-based behavioral interventions like motivational interviewing and problem-solving.
  • Monitor clinical symptoms and document progress within the EHR system.
  • Collaborate with psychiatric consultants to adjust treatment plans for patients.
  • Facilitate referrals to external social and specialty care services.

Requirements

  • Active RN or LCSW licensure required.
  • Minimum of 2 years of experience in direct case or care management.
  • Background in ALS, Parkinson’s, or chronic neurodegenerative disease care.
  • Knowledge of insurance processes and medical billing/coding.
  • Experience reviewing medical records and working with clinical trials.
  • Ability to thrive in a fast-growing startup environment.
