What you will do

• Review caseload and prepare technology for virtual patient visits.
• Partner with clinical in-home team members (Sprinters) for patient visits.
• Conduct health risk assessments, medication reviews, and cognitive screenings.
• Provide patient education regarding health and well-being.
• Collaborate with Sprinters to evaluate vital signs and arrange blood draws.
• Provide comprehensive care coordination and management, including referrals and documentation.

Requirements

• Board Certified as a Family Nurse Practitioner in California.
• Active Family Nurse Practitioner License in California.
• Consistently exhibits professionalism, integrity, and compassion.
