What you will do

• Review caseload and prepare technology for virtual patient visits.
• Partner with clinical in-home team members (Sprinters) for patient visits.
• Make connections with a diverse range of patients, collecting relevant information.
• Conduct health risk assessments, medication reviews, and cognitive screenings.
• Evaluate vital signs and arrange blood draws in collaboration with Sprinters.
• Provide comprehensive care coordination and management, including referrals.

Requirements

• Board Certified as a Family Nurse Practitioner in TX, LA, and/or OK Licensed
• Active Family Nurse Practitioner License in California
• Consistently exhibits professionalism, integrity, and compassion.
• Willingness to work in a revolutionary environment requiring problem-solving.
• Technologically savvy and comfortable using laptops and secure messaging apps.
• Strong written and verbal communication skills.
