Location: Must be based in the USA and hold active NP or PA licensure in Ohio, North Carolina, South Carolina, Texas, Alabama, or Illinois.

Salary: $120k–$155k base + performance bonus (total comp $130k–$170k)

What you will do

• Deliver direct nephrology care, including medication optimization and volume status management.
• Collaborate with physicians, pharmacists, and social workers to coordinate holistic, interprofessional care plans.
• Conduct comprehensive clinical assessments, including diagnosis review and quality of care interventions (STARS/HEDIS).
• Facilitate shared decision-making regarding goals of care and dialysis initiation planning.
• Use electronic health information systems to document visits and provide feedback for product improvement.
• Participate in panel review discussions and ongoing quality improvement initiatives.

Requirements

• Active RN and APRN license in OH, NC, SC, TX, AL, or IL (at time of hire and maintained throughout employment).
• Master's or Doctoral degree in Nursing with a specialization in nephrology.
• Minimum 2 years of experience in a nephrology practice or dialysis unit.
• 5+ years of experience in an outpatient clinical practice.
• Proficiency in telehealth technology and EHR systems.
• Active BLS certification.

Nice to have

• Bilingual in English and Spanish.
• Prior experience in virtual care environments.
• Understanding of managed care, including STARS/HEDIS measures and clinical coding.
