Specialty Care Navigator (Trauma): Managing intake, assessment, and care coordination for adult and adolescent members with PTSD and complex trauma with an accent on evidence-based trauma models and family-centered support. Focus on providing high-touch clinical triage, ongoing stabilization, and seamless referral integration within a multidisciplinary mental health platform.
Location: Must be based in the US. Shift: Monday-Friday, 9-5pm PT

What you will do
- Conduct telephonic and telehealth screenings to assess member needs and determine appropriate levels of care.
- Apply motivational interviewing and active listening to support members in goal setting and stabilization.
- Coordinate trauma-informed care pathways including referrals to internal providers and external community resources.
- Manage family dynamics by engaging with adolescents and caregivers to support comprehensive treatment plans.
- Maintain accurate patient records and documentation within the Electronic Health Record (EHR) system.
- Provide in-the-moment safety planning and crisis intervention for members at risk.
Requirements
- Independent clinical mental health license required (LCSW, LMFT, LPC, or equivalent).
- Minimum 2+ years of clinical experience with PTSD and Complex Trauma in adult and adolescent populations.
- Availability to work Monday-Friday, 9-5pm PT.
- Proficiency in phased-based trauma treatment models and evidence-based practices (e.g., CPT, EMDR).
- Strong background in diagnostic assessment, triage, and motivational interviewing.
- Exceptional verbal and written communication skills with comfort in a tech-forward clinical environment.
