Case Management Coord II (RN)

Location: National remote within the continental United States; California, New York, Alaska, and Hawaii are excluded. Ability and willingness to travel within the assigned location is required.

Annual salary: $69,800–$125,900, depending on skills, education, knowledge, experience, and location.

What you will do

• Conduct telephonic outreach to recently discharged and high-risk patients.
• Facilitate discharge coordination and identify alternate treatment programs.
• Assess, plan, implement, coordinate, monitor, and evaluate health care options and services.
• Consult with physicians, providers, members, and other relevant resources.
• Serve as a liaison between physicians and members while supporting quality and cost-effective outcomes.

Requirements

• Registered nurse qualification is required for the RN position.
• Ability to provide telephonic support for patients across all locations.
• Must be located within the continental United States; California, New York, Alaska, and Hawaii are excluded.
• Ability and willingness to travel within the assigned location.
