Case Manager (Healthcare)

Location: Remote Flex - North Carolina, United States; virtual work with a few in-office visits each year depending on distance from the Durham headquarters. Must work Monday-Friday between 8:00am and 7:00pm EST.

Salary: $73,698-$117,917 annually, plus an annual incentive bonus and benefits.

What you will do

• Coordinate care, resources, and services for members as part of a multidisciplinary healthcare team.
• Perform comprehensive clinical assessments and develop member-centered treatment, intervention, and discharge plans.
• Monitor member progress, conduct follow-ups, provide education, make referrals, and modify care goals as needed.
• Evaluate and facilitate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Assess medical necessity, treatment alternatives, benefit options, and barriers to achieving care goals.
• Document assessments, care plans, interventions, monitoring activities, and outcomes; develop procedures or training when needed.

Requirements

• Registered Nurse with at least three years of clinical and/or case management experience.
• Valid North Carolina or compact multi-state clinical licensure required.
• Additional specialty certification such as CCM or CDCES required within two years of hire.
• Must be able to work Monday-Friday between 8:00am and 7:00pm EST.
• Employment authorization sponsorship is not available.
