Manager, Care Management (Healthcare)

Location: Remote within Illinois, limited to Cook County ZIP codes or within a 10-mile radius of the listed ZIP codes; requires travel throughout Illinois and occasional onsite meetings at hirify.global locations.

Salary: $94,900-$130,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead and supervise a diverse team of nurses, care coordinators, and behavioral health professionals, with up to 15 direct reports.
• Oversee case management, disease management, post-discharge calls, discharge planning, and elective admission pre-assessments.
• Identify members for care management activities and monitor care coordination effectiveness and service performance.
• Analyze trends, develop system-level recommendations, and create reporting tools for clinical performance.
• Develop audit plans and tools to support compliance with state contracts and member care requirements.
• Recruit, onboard, mentor, coach, evaluate, and manage the performance of care coordination associates.

Requirements

• Must reside in the specified Cook County, Illinois ZIP codes or within a 10-mile radius.
• Hold an active, unrestricted Illinois RN, LCSW, or LCPC license.
• Have at least five years of professional healthcare or care management experience and at least two years of leadership experience.
• Use electronic medical records and other electronic information systems effectively.
• Analyze and interpret data trends.
• Travel up to 25% throughout Illinois for field-based care team oversight visits, member and family meetings, community partner meetings, and other care activities.
• Work Monday-Friday, generally 8:00 AM-5:00 PM CST, with schedule flexibility based on business needs.

Nice to have

• Advanced degree in nursing or a healthcare business field.
• Certified Case Manager certification.
• Managed care experience and five or more years of management or supervisory experience.
• Experience with STARS, HEDIS, healthcare quality measures, or clinical program evaluation.
• Knowledge of community health, social service agencies, and community resources.
• Bilingual or multilingual ability in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language.
