Onsite Care Coordination Nurse

Location: Hybrid/onsite at Aurora St. Luke's Medical Center in Milwaukee, Wisconsin; must live within a 30–60 minute driving distance. Work includes in-person facility engagement and limited remote documentation from a private workspace. Monday–Friday, 8:00 AM–5:00 PM local time; no weekends, holidays, or on-call work. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Coordinate discharge planning from admission through post-discharge and facilitate follow-up visits with primary care providers.
• Identify care gaps, safety concerns, and discharge barriers, then implement strategies to address them.
• Collaborate with attending providers, facility case managers, and other healthcare professionals.
• Educate members and families about diagnoses, medications, preventive care, and ongoing wellness.
• Complete documentation and follow infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Wisconsin Registered Nurse license required.
• Must live within a 30–60 minute driving distance of the assigned Milwaukee facility.
• At least three years of recent acute care nursing experience.
• Experience with hospital or health-plan discharge planning or care coordination.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver's license, dependable automobile, vehicle liability insurance, TB screening, background clearance, and facility credentialing.

Nice to have

• Bachelor's degree in Nursing.
• Care management, health promotion, coaching, utilization management, or health plan experience.
• Care management certification and knowledge of HEDIS and Stars measures.
• Bilingual English/Spanish communication skills.
