Care Navigator - OB

Location: Remote from anywhere within the state of Missouri, United States. Monday-Friday, 8am-5pm; occasional on-site visits may be required.

Pay range: $22.94-$38.79 per hour.

What you will do

• Evaluate member needs, barriers to care, available resources, and recommended care outcomes.
• Develop or support personalized care plans and identify appropriate providers, specialists, and community resources.
• Educate pregnant members, families, and caregivers about care options, healthcare benefits, referrals, and provider instructions.
• Coordinate access to care between members, caregivers, healthcare providers, care managers, and community resources.
• Monitor care-plan progress and member status, collaborate on revisions, and refer members for further care-management evaluation when appropriate.
• Document member information and care-management activities in compliance with state, federal, and payer regulations; perform on-site assessments when appropriate.

Requirements

• Bachelor’s degree and 2-4 years of related experience, or equivalent applicable knowledge and experience.
• Graduation from an accredited school of nursing is required if holding clinical licensure.
• Applicants must be able to work remotely from within Missouri.
• Current state clinical license preferred; a Missouri LPN license is strongly preferred.
