FEP Case Manager (Healthcare)

Location: Remote Flex - North Carolina; candidates may also work from South Carolina, Virginia, Tennessee, Maryland, Georgia, Ohio, or Kentucky. Remote Flex work is virtual with a few in-office visits each year.

Salary: $73,698.00–$117,917.00 annually

What you will do

• Coordinate care, resources, and services for members as part of a multidisciplinary healthcare team.
• Conduct comprehensive assessments and develop member-centered treatment, intervention, and discharge plans.
• Monitor member progress, arrange referrals and education, identify barriers, and update care goals as needed.
• Evaluate and facilitate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Support high-risk, high-cost, or high-utilization members and assess medical necessity, benefits, and alternative treatment plans.
• Document assessments, care plans, interventions, monitoring, outcomes, and related policies or training materials.

Requirements

• Registered Nurse with at least three years of clinical and/or case management experience.
• Valid North Carolina or compact multi-state clinical licensure is required and must be maintained.
• Certified Case Manager certification must be obtained within two years of employment.
• Ability to perform clinical assessments, care coordination, member education, and documentation.

Nice to have

• Strong technical aptitude and ability to learn new systems quickly.
