Government Programs Care Manager II – 743 – Work From Home

Location: Remote within the continental United States, excluding California, New York, Alaska, and Hawaii. Field visits to members' homes or facilities and travel within Cook and Collar counties are required. Work hours are 8:00 AM–4:30 PM CST, Monday through Friday.

Base pay: $28.92–$47.50

What you will do

• Provide telephonic and field-based care coordination for government program members.
• Educate members about health care programs and support adherence to personal health goals.
• Conduct telephonic and field-based health screenings and comprehensive health assessments.
• Develop care plans and respond to member inquiries.
• Coordinate activities with providers, internal clinical operations, external vendors, and community resources.
• Travel to members' homes or facilities within the assigned territory.

Requirements

• Hold an unrestricted RN, LPC, LMSW, LCPC, LCSW, LMFT, LPCC, LISW, LSW, or LMHC license applicable to the state of operations.
• Have at least 3 years of wellness or managed care experience presenting clinical issues to members and physicians.
• Demonstrate knowledge of the health and wellness marketplace and employer trends.
• Have verbal and written communication, medical data analysis, PC, database, and web application skills.
• Hold a current unrestricted driver's license, transportation, and applicable insurance.
• Be able and willing to travel within the assigned territory and work remotely from the continental United States, excluding California, New York, Alaska, and Hawaii.

Nice to have

• Direct clinical, patient education, condition management, or government programs experience.
• Experience managing complex or catastrophic cases.
• Case management, training, project management, or nationally recognized health care certification.
• Bilingual English and Spanish skills.
• For Illinois high-needs children programs: a bachelor's degree in nursing, social sciences, social work, or a related field and one year of supervised clinical experience in a human-services field.
