LTSS Service Care Coordinator

Location: Remote in Illinois with approximately 50% travel throughout DuPage, Kane, Kendall, and Will Counties; candidates must live within 30 minutes of the service area. The remaining time is remote, with in-person member visits and community-based support required.

Pay: $22.94–$38.79 per hour

What you will do

• Evaluate member needs, available resources, and appropriate care outcomes.
• Develop and monitor long-term care plans and service plans.
• Coordinate access to providers, specialists, community resources, and support services.
• Educate members and families or caregivers about healthcare instructions, service options, referrals, and benefits.
• Conduct in-person assessments and collaborate with healthcare providers and community partners.
• Document member information and care management activities in compliance with regulatory requirements.

Requirements

• Bachelor’s degree and one year of related experience, or equivalent applicable experience.
• Previous case management experience is highly preferred.
• Must live within 30 minutes of the Illinois service area and be able to travel throughout DuPage, Kane, Kendall, and Will Counties.
• For the Illinois Plan, candidates working with physically disabled or elderly members must be an Illinois-licensed RN, hold a qualifying human services-related bachelor’s or master’s degree, or be an LPN with one year of relevant assessment and formal service experience.
• Ability to coordinate with members, families, caregivers, providers, and community resources.
