Complex Care Specialist

Location: Remote within the United States. Work cannot be completed from outside the United States. Required schedule: Monday–Friday, 9:00 a.m.–5:30 p.m. Pacific Time, with participation in an on-call rotation.

Salary: $38.65 per hour base rate.

What you will do

• Manage a caseload of complex, multi-need clients through long-term case management.
• Assess risk and clinical needs, create safety plans, de-escalate crises, and connect clients with appropriate levels of care.
• Support treatment engagement using Motivational Interviewing and evidence-based coping strategies.
• Identify treatment options using matching software, facility databases, health plan directories, and internet research.
• Develop personalized care plans, monitor PHQ-9 and GAD-7 symptoms, and present cases at multidisciplinary meetings.
• Coordinate care with facilities, health plans, navigators, billing teams, and other providers, including discharge planning and DBT screenings.

Requirements

• Master’s degree in clinical social work or an equivalent qualification.
• LCSW or equivalent independent clinical license, such as LMFT, LPC, or LPCC, is required; supervised hours are not provided.
• Significant experience managing high-risk clinical situations, including suicidality and mandated reporting.
• Experience with case management, health plans, medical necessity criteria, higher levels of care, and community mental health resources.
• Ability to work independently in a remote environment and collaborate with a clinical team.
• Full-time United States residency and availability for the specified Pacific Time shift are required.
