Clinical Social Worker, Triage Team (11:30AM-8:00 PM EST)

Location: Remote; working hours are 11:30 AM–8:00 PM EST

Base salary: $92,000 per year; the range may vary depending on the hiring level, qualifications, experience, and internal equity.

What you will do

• Serve as the primary intake clinician for escalated behavioral health and advanced social-needs consultations.
• Conduct psychosocial, behavioral health, safety, and risk assessments by phone, video, or ThymeBox.
• Deliver brief interventions, de-escalation, suicide risk management, and immediate safety planning.
• Coordinate with nurses, care partners, providers, and community organizations to address housing, financial, legal, custody, caregiver, and care-access barriers.
• Document assessments, safety plans, referrals, and escalations; participate in case conferences and clinical huddles.
• Monitor triage trends and help improve workflows, triage tools, decision support, and pod handoffs.

Requirements

• Master’s degree in Social Work from a CSWE-accredited program.
• Valid, active, unrestricted independent clinical social work license in the current state of practice; willingness to obtain additional state licenses for cross-state practice.
• At least 3 years of clinical social work experience in a healthcare setting.
• Experience with risk assessment, crisis management, suicidality, homicidality, psychosis, and time-sensitive clinical decisions.
• Strong telehealth and phone-based clinical skills, documentation, communication, and collaboration abilities.
• Comfort using video visits, electronic health records, care-management platforms, Google Suite, Slack, or similar technology.

Nice to have

• Oncology, palliative care, serious illness, hospice, behavioral health, or care-navigation experience.
• OSW-C or APHSW-C certification.
• Experience as a subject-matter consultant, in a Behavioral Health pod model, or in a startup, virtual-care, or technology-focused environment.
