Senior Care Manager (Behavioral Health)

Location: Remote-NY; candidates must be licensed in New York State. Schedule: Monday–Friday, 8:30 a.m. to 5:00 p.m. ET.

Salary: $75,300–$135,400 per year.

What you will do

• Develop and continuously assess personalized care plans for high-acuity members with complex behavioral health and social needs.
• Conduct telephonic, digital, home, and other site-based outreach to assess needs and coordinate services.
• Coordinate members, families, healthcare providers, specialists, and community resources to ensure timely access to care.
• Monitor treatment outcomes, clinical symptoms, complications, side effects, and care gaps, revising plans when needed.
• Manage a high-volume caseload of complex and specialty cases involving mental health and substance use disorders.
• Support clinical new hires through precepting, coaching, onboarding, competency checkpoints, and readiness assessments.

Requirements

• New York State licensure is required.
• Licensed Master’s Behavioral Health Professional credential, such as LCSW, LMSW, LMFT, LMHC, or LPC, or an RN license based on state contract requirements.
• Master’s degree in behavioral health or social work, or a degree from an accredited school of nursing.
• Four to six years of related experience and behavioral health experience.
• Strong clinical judgment, active listening, prioritization, adaptability, and resourcefulness.
• Ability to work across multiple healthcare and community systems while maintaining accurate regulatory documentation.
