Field Care Manager Behavioral Health

Location: Remote, field-based position; must reside in South Carolina and serve an assigned South Carolina area. At least 50% local travel and periodic travel to the hirify.global Columbia office are required.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face assessments of enrollees’ behavioral, physical, environmental, and psychosocial needs.
• Develop individualized care plans and connect enrollees and families with clinical, community, and social resources.
• Identify barriers to care, monitor progress, and adjust interventions to support desired outcomes.
• Coordinate holistic support with providers, community-based services, community health workers, housing specialists, and care management assistants.
• Use clinical judgment to determine when in-person support is appropriate and coordinate field-based meetings.

Requirements

• Must reside in South Carolina.
• Active Registered Nurse license or licensed master-level social worker credential, such as LCSW, LMSW, LPC, or LMFT.
• At least one year of behavioral health clinical experience.
• At least two years of recent case management or care management experience.
• Experience coordinating services, care needs, or benefits for Medicaid and/or Medicare enrollees.
• Knowledge of community health and social service agencies, Microsoft Office applications, and PHI/HIPAA privacy requirements.

Nice to have

• Experience in community mental health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
