Field Care Management Behavioral Health - Field/Remote LA (Behavioral Health)

Location: Remote within Louisiana Region 7: Bienville, Bossier, Caddo, Claiborne, DeSoto, Natchitoches, Red River, Sabine, or Webster. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Perform telephonic and face-to-face behavioral health assessments and evaluations.
• Guide members and families to appropriate care, community, and support resources.
• Create care plans and monitor progress toward desired outcomes.
• Coordinate behavioral health, physical health, social determinant of health, and value-added services.
• Collaborate with providers, community services, primary care providers, and the transdisciplinary care team.
• Coordinate transitions of care, identify barriers, and submit incident reports.

Requirements

• Must reside in Louisiana Region 7: Bienville, Bossier, Caddo, Claiborne, DeSoto, Natchitoches, Red River, Sabine, or Webster.
• LCSW, LPC, LAC, LMFT, or RN license/qualification.
• RN candidates must have at least 2 years of psychiatric experience.
• Passion for improving consumer experiences and outcomes.
• Patient-facing work requires participation in hirify.global’s tuberculosis screening program.
• Work from a dedicated space that protects member PHI and HIPAA information; home internet must provide at least 25 Mbps download and 10 Mbps upload speeds.
