Field Care Manager, Social Worker

Location: Must reside in Michigan, with the ability to travel to Wayne and Macomb counties. Hybrid work style: 80% remote and 20% field, with travel of up to 20%. Schedule follows Eastern Time, Monday through Friday, either 8:30 AM–5:00 PM or 9:00 AM–5:30 PM.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Manage a caseload and conduct comprehensive assessments in members’ homes, community settings, and by telephone.
• Develop and coordinate individualized care plans for members with medical and behavioral health needs.
• Collaborate with interdisciplinary care teams, caregivers, primary care providers, health coaches, and external resources.
• Identify barriers to care and connect members with social, housing, educational, and community services.
• Review medical attestations, provide clinical oversight, and ensure care is delivered in the least restrictive setting.
• Serve as the primary point of contact for coordinated care delivery.

Requirements

• Residence in Michigan and ability to travel to Wayne and Macomb counties are required.
• Bachelor’s degree in Social Work from an accredited institution.
• Active Michigan social work licensure in good standing, including LLBSW, LBSW, LLMSW, LMSW, or LCSW; limited license holders must be progressing toward full licensure.
• At least two years of post-degree clinical or care management experience in behavioral health, medical, or community-based settings.
• Experience with adult populations, chronic conditions, disease management, and community health resources.
• Valid driver’s license, reliable transportation, communication skills, independent working ability, and proficiency with electronic medical records and Microsoft Word, Outlook, and Excel.

Nice to have

• LMSW or LCSW qualification.
• Experience with in-home assessment, care coordination, health promotion, wellness coaching, Medicaid Long Term Care, or managed care.
• Bilingual Spanish, Arabic, or Chaldean Neo-Aramaic skills.
• Motivational Interviewing certification or knowledge.
