Transition of Care - RN Care Manager

Location: Remote-KY; preference for applicants residing in Kentucky, or in the Eastern or Central Time Zone. Schedule: Monday through Friday, 8:00 AM–5:00 PM CT or ET.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, barriers to care, and social determinants of health, then develop personalized care and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources to address unmet needs.
• Monitor member status, changes in condition, and progress toward care plan goals, updating plans as needed.
• Provide education on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Conduct telephonic, digital, home, and other site outreach to assess needs and coordinate services.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor's degree in Nursing.
• 2–4 years of related experience in medical-surgical or inpatient units, home health, outpatient clinical settings, or care management.
• Active Kentucky State or Compact RN license required.
• Applicants should reside in Kentucky or the Eastern or Central Time Zone.
