Transitions of Care Advanced Practice Provider (NP or PA)

Location: Remote USA. Full-time schedule is Monday-Friday, 8:30 AM-5:00 PM, with CST/MST/PST preferred.

Salary: $120,000-$150,000 per year base salary, plus a performance-based bonus of $10,000-$20,000 annually or quarterly; total compensation range of $130,000-$170,000 per year.

What you will do

• Conduct comprehensive virtual post-discharge visits covering medical conditions, medications, functional status, and psychosocial needs.
• Perform medication reconciliation, optimize guideline-directed therapy, place prescriptions and clinical orders, and provide education and contingency plans.
• Assess and manage acute or worsening symptoms in medically complex patients, determine the appropriate level of care, and escalate when needed.
• Manage patient care plans throughout the 30-day post-hospitalization period, including chronic disease and symptom management.
• Collaborate in interdisciplinary rounds with case managers, care coordinators, social workers, pharmacists, primary care providers, and specialists.
• Support clinical education, quality-improvement initiatives, EHR feedback, team communication, and the ongoing development of the TOC program.

Requirements

• 5+ years of direct patient-care experience, primarily with geriatric populations and multiple chronic, complex, comorbid conditions.
• Experience managing both acute presentations and longitudinal outpatient chronic disease across the care continuum.
• Active, unrestricted RN and APRN license or Physician Assistant license in good standing.
• Ability and willingness to obtain licenses in required Devoted states within 90 days of hire.
• Ability to work full time, five 8-hour days per week, Monday-Friday.

Nice to have

• Experience in internal medicine, urgent care, emergency medicine, hospital medicine, post-acute medicine, or geriatrics.
• Telehealth video visit experience and experience managing post-transition patients.
• Experience treating CHF exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies.
• Experience working in interdisciplinary teams.
