FCP RN Care Manager (Healthcare)

Location: Milwaukee, Wisconsin, with home-based work and field visits throughout Milwaukee and surrounding counties

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan

What you will do

• Complete nursing assessments and provide clinical input for member care plans.
• Monitor member wellness, health promotion activities, ongoing conditions, and responses to interventions.
• Conduct home and community visits and travel up to 50% throughout Milwaukee and surrounding counties.
• Assess members’ ability to manage medications, treatments, medical equipment, and access to care.
• Arrange and monitor healthcare services, supports, and interventions based on identified needs.
• Respond to psychiatric, behavioral, and medical crises in coordination with other staff and maintain member safety.

Requirements

• Active Registered Nurse license in good standing in the State of Wisconsin.
• Two years of related healthcare experience.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Ability to work Monday–Friday, 8:30 a.m.–5:00 p.m. Central Standard Time.
• Ability to work from a dedicated home workspace that protects member PHI and HIPAA information.
• Ability to meet the minimum home internet requirements of 25 Mbps download and 10 Mbps upload.

Nice to have

• Bachelor of Science in Nursing degree.
• Home health, managed care, or case management experience.
• Knowledge of Medicaid and Medicare benefits.
