Bilingual Spanish/English Transitions of Care Advanced Practice Provider (NP or PA)

Location: Remote USA. Full-time schedule is Monday–Friday, 8:30 AM–5:00 PM, with CST/MST/PST preferred. Licensure must be active in required Devoted states, with willingness to obtain additional state licenses within 90 days of hire.

Salary: $120,000–$150,000 per year base salary, plus a performance-based bonus of $10,000–$20,000 annually; total compensation range of $130,000–$170,000 per year.

What you will do

• Conduct comprehensive initial transitions-of-care visits after hospitalization, including medical, medication, functional, and psychosocial assessments.
• Reconcile medications, optimize guideline-directed therapy, place orders, arrange referrals and services, and provide disease education and contingency plans.
• Assess and manage acute or worsening symptoms virtually, determine the appropriate level of care, and escalate when necessary.
• Manage patients’ clinical trajectories throughout the 30-day post-discharge period, including chronic disease and symptom management.
• Collaborate with case managers, care coordinators, social workers, pharmacy, primary care providers, and specialists during interdisciplinary rounds.
• Support team education, quality-improvement projects, program development, and feedback on the organization’s EHR.

Requirements

• Bilingual Spanish/English communication skills required.
• At least 5 years of direct patient-care experience, primarily with geriatric patients who have multiple complex comorbidities.
• Experience managing both acute presentations and longitudinal outpatient chronic disease.
• Active, unrestricted RN/APRN or Physician Assistant licensure in good standing, with eligibility for required Devoted states.
• Ability to work full-time during the stated weekday schedule and provide virtual care.

Nice to have

• Experience in internal medicine, urgent care, emergency medicine, hospital medicine, post-acute medicine, or geriatrics.
• Telehealth video experience and experience managing post-discharge patients.
• Experience with CHF, diabetes, COPD, and hypertensive emergencies or exacerbations.
• Experience working in interdisciplinary clinical teams.
