LTSS Service Care Coordinator (Healthcare)

Location: Remote-IL, with residence and regular travel required within the designated Illinois service area. The role involves approximately 75% travel, including in-home and on-site visits.

Salary: $22.94–$38.79 per hour

What you will do

• Evaluate member needs, available resources, and potential care outcomes.
• Develop and monitor personalized long-term care and service plans.
• Coordinate services among members, families, caregivers, providers, specialists, and community resources.
• Conduct in-home and on-site visits, assess needs, and support access to services.
• Educate members and caregivers about healthcare benefits, procedures, referrals, and service options.
• Document care management activities and provide feedback on quality and cost-effective service delivery.

Requirements

• Bachelor’s degree and one year of related experience, or equivalent relevant experience.
• Prior case management experience is highly preferred.
• Reliable transportation and ability to travel regularly within the designated service area.
• For the Illinois plan, candidates serving physically disabled or elderly members must be an Illinois-licensed RN, hold a qualifying human services-related bachelor’s or master’s degree, or be an LPN with one year of relevant assessment and formal service experience.
