Foster Care Liaison - Care Navigator

Location: Remote in Illinois with approximately 25% local travel to DCFS and Child Welfare Contributing Agency offices in northern Cook County. Applicants must reside in Illinois, with preference for Cook County, especially near Maywood, Hillside, Deerfield, Lincolnwood, Arlington Heights, Evanston, Niles, and Elmhurst. Some on-site visits and weekend events are required.

Pay range: $22.94–$38.79 per hour.

What you will do

• Assess member needs, barriers to care, available resources, and appropriate support options.
• Develop and coordinate personalized care plans and service plans, including provider, specialist, and community resource referrals.
• Provide psychosocial and resource support related to housing, food, education, employment, independent living, justice, and foster care.
• Coordinate communication between members, caregivers, healthcare providers, care managers, DCFS staff, and other stakeholders.
• Conduct local office and member visits, monitor progress toward care goals, and support care plan revisions.
• Provide education, administrative coordination, stakeholder training, and immediate problem resolution for DCFS and YouthCare partners.

Requirements

• Must reside in Illinois.
• Bachelor’s degree in Social Work, Nursing, Health Behavioral Science, or equivalent experience.
• At least 2 years of experience in a social service agency or advocacy environment.
• Ability to travel locally to DCFS and child welfare offices and perform occasional on-site visits.
• Availability for Monday-Friday schedules aligned with Central Time; some weekend events may be required.
• Ability to document care management activities and comply with state, federal, and payer regulations.
