Care Coach

Location: Must reside in South Carolina. Remote, field-based position serving members throughout an assigned South Carolina service area, with at least 50% regional travel for face-to-face meetings and field interactions. A dedicated lockable home office is required to protect member privacy and comply with PHI and HIPAA requirements.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face assessments of members’ health, functional, environmental, psychosocial, and support needs.
• Develop and maintain individualized care plans based on member goals, preferences, and appropriate care settings.
• Coordinate services, implement interventions, monitor progress, and resolve barriers to care and engagement.
• Connect members and families with clinical, community, social, and payer resources while collaborating with providers and interdisciplinary teams.
• Support transitions across care settings and visit members in homes, assisted living facilities, long-term care facilities, and community locations.
• Document assessments, care plans, referrals, interactions, and outcomes accurately in required systems.

Requirements

• Must reside in South Carolina and be able to travel at least 50% regionally.
• Bachelor’s degree in a health and human services field.
• At least two years of relevant experience in care coordination, case management, health and human services, or a related field.
• Experience assessing needs, developing care plans, coordinating resources, and documenting member outcomes.
• Intermediate to advanced proficiency with Microsoft Word, Excel, and Outlook.
• Completion of tuberculosis screening is required, as well as a dedicated lockable home office for protecting member information.

Nice to have

• Master’s degree in a health and human services field.
• Applicable state license or Certified Case Manager credential.
• Experience with InterQual or MCG guidelines and Medicare or Medicaid populations.
• Experience with electronic case-note documentation, health promotion, coaching, wellness, and community resources.
