Manager Social Work Case Management (Healthcare)

Location: Remote work from Pennsylvania, United States; physical work site required

What you will do

• Lead hiring, coaching, development, performance management, recognition, and productivity for a team of licensed clinical professionals.
• Direct daily department operations, including staffing, scheduling, policies, programs, and potential budget responsibilities.
• Provide clinical oversight for psychosocial assessments, care plans, interventions, documentation, member advocacy, and evidence-based social work practices.
• Act as a clinical escalation resource for complex psychosocial cases and support motivational interviewing and member engagement practices.
• Monitor engagement, care plan completion, quality, outcomes, member satisfaction, and cost-of-care metrics while identifying improvement and cost-saving opportunities.
• Collaborate with clinical, operational, provider, quality, product, analytics, and client-facing stakeholders on enterprise initiatives and regulatory assessments.

Requirements

• Master's degree in social work, social welfare, counseling, psychology, public health, healthcare administration, or another clinically relevant field.
• At least 5 years of health-related clinical work experience and 3 years of management, supervisory, team lead, or project leadership experience.
• Current licensure as an LSW, LCSW, LPC, LMFT, state RN, or through the enhanced Nurse Licensure Compact (eNLC).
• Experience with social work, care management, population health, healthcare operations, diverse populations, and healthcare systems.
• Ability to lead a remote or geographically dispersed workforce and maintain engagement, accountability, communication, and productivity in a virtual environment.
• Must work from Pennsylvania, United States.

Nice to have

• MS in Social Work or Social Welfare.
• Health plan or payer experience and telephonic care delivery experience.
• LSW or LCSW licensure and Certified Case Manager (CCM) certification.
• Lean/Six Sigma training or certification.
