Care Manager (Healthcare)

Location: Remote within Michigan; candidates must be based in Michigan. Travel of up to 10% may be required to office practices and staff retreats within Michigan.

Salary: $70,000–$87,000 base per year, plus bonus and equity.

What you will do

• Provide telephone-based health coaching, education, self-management support, care coordination, and transition planning for patients and caregivers.
• Assess medical, behavioral, pharmaceutical, and social needs while identifying care gaps and barriers to improved health.
• Collaborate with primary care physicians and interdisciplinary teams to identify high-risk patients and develop appropriate care plans.
• Use the hirify.global App and real-time population health data to manage high-risk patients and intervene with high utilizers.
• Support population health initiatives, improve clinical, financial, and functional outcomes, and reduce unnecessary hospital and specialist utilization.
• Provide clinical oversight to non-licensed support staff and delegate appropriate supportive tasks.

Requirements

• Active, unencumbered Michigan license as a Registered Nurse or fully licensed Master Social Worker.
• Must be a resident of Michigan.
• 3–5 years of direct healthcare experience, preferably in home health, ambulatory care, community public health, case management, or care coordination.
• Experience with patient education, self-management support, care coordination, or vulnerable populations.
• Ability to document competently in electronic health records and communicate effectively in writing and verbally.
• Ability to work sedentary hours on the telephone and computer, with proficient communicative, auditory, and visual skills.

Nice to have

• Population health, telephonic care management, or remote managed care experience.
• Understanding of quality metrics and experience activating patients.
• Familiarity with the local healthcare community and experience working with geriatric, minority, or behavioral health populations.
• Structured problem-solving skills, curiosity, receptiveness to feedback, and a collaborative working style.
