TL;DR
LTSS Service Care Manager - Behavioral Health, STAR Kids (Behavioral Health): Coordinating holistic care plans and community services for high-risk pediatric Medicaid members with significant mental and behavioral health needs, with an accent on home-based assessments, family education, and provider collaboration. Focus on conducting frequent home visits, managing crisis and de-escalation needs, coordinating behavioral and social services, and maintaining compliant clinical documentation.

Location: Remote-TX with in-person member engagement across Central Austin, South Austin, Southeast Austin, or Northwest Austin, Texas; up to 75% monthly travel for home visits is required. Standard schedule is Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility for pediatric members' school schedules.

Salary: $27.02–$48.55 per hour.

What you will do

  • Assess complex and high-risk pediatric members with mental and behavioral health needs and develop individualized care or service plans.
  • Coordinate medical, behavioral, and social services, including housing, transportation, employment, foster care, meals, and activities of daily living.
  • Act as a liaison and advocate for members and families while collaborating with physicians, facilities, agencies, providers, and community resources.
  • Conduct frequent home and site visits, provide member and caregiver education, and support referrals and service authorizations.
  • Provide behavioral health expertise for crisis calls, mental health first aid, field safety, de-escalation, and medication monitoring.
  • Maintain accurate electronic care management documentation and support compliance with clinical, state, and federal guidelines.

Requirements

  • Master's degree in Mental Health or Social Work, or graduation from an accredited school of nursing.
  • 2–4 years of related behavioral or mental health experience.
  • An unrestricted behavioral health professional license required for the applicable state contract, such as LPC, LCSW, LMFT, LMHC, or RN with direct behavioral health experience.
  • Experience with behavioral health case management, high-risk caseloads, community resources, clinical documentation, and care coordination.
  • Proficiency with web-based applications and case management systems, including TruCare and Microsoft Office applications.
  • Ability to work independently, manage changing needs, and communicate effectively with members, families, providers, and leadership.

Nice to have

  • 5+ years of behavioral health case management experience, especially in psychiatric hospitals, community counseling, schools, residential treatment, or youth-focused behavioral health organizations.
  • Direct field-based case management or social work experience.
  • Experience in managed care or federal and state-sponsored government programs.
  • Bilingual English/Spanish skills.
