Senior Care Manager (Behavioral Health)

Location: West Allis, Wisconsin, with approximately 50% field-based visits throughout the Milwaukee area; hybrid schedule with phone, in-home, and other site visits.

Salary: $75,300–$135,400 per year

What you will do

• Develop, implement, and continuously assess personalized care and service plans for members with complex and high-acuity behavioral health and social needs.
• Conduct telephonic, digital, in-home, and site-visit outreach to assess needs, monitor treatment outcomes, and identify changes in clinical symptoms, complications, or side effects.
• Coordinate access to healthcare providers, behavioral health specialists, mental health services, substance use resources, and community programs.
• Review referrals, member data, health risks, and care gaps to recommend appropriate interventions and improve quality outcomes.
• Educate members and families about treatment options, provider instructions, referrals, procedures, benefits, and available services.
• Support leadership and clinical teams with quality improvement, regulatory compliance, and precepting or coaching of new clinical hires.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 4–6 years of related experience in case management, managed care, or complex care coordination.
• Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN according to state contract requirements.
• Behavioral health experience is required, including familiarity with mental health and substance use disorders.
• Experience with HMO and/or Medicaid plans and knowledge of programs such as SSI and TANF is preferred for the role.
• Ability to perform approximately 50% field-based visits in the Milwaukee area is required.
