Care Navigator

Location: Remote, field-based; applicants must reside in Oklahoma, United States. Monday through Friday, 8:00 AM to 5:00 PM, with possible on-site member visits.

Salary: $22.94–$38.79 per hour

What you will do

• Evaluate member needs, barriers to care, available resources, and the best care outcomes.
• Develop or contribute to personalized care plans and identify providers, specialists, and community resources.
• Coordinate members, families, caregivers, healthcare providers, and care managers to ensure timely access to services.
• Provide psychosocial support, education, referrals, and assistance with resources such as housing, food, education, employment, foster care, and independent living.
• Monitor progress toward care-plan goals, document care-management activities, and support plan revisions when member needs change.
• Conduct on-site visits when appropriate and maintain compliance with applicable regulatory requirements.

Requirements

• Must reside in Oklahoma, United States.
• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable experience.
• Experience with children and families, behavioral or mental health, assessments, community referrals, care coordination, and problem-solving.
• Strong communication, customer service, computer, organizational, and time-management skills.
• Graduation from an accredited school of nursing is required for candidates holding clinical licensure.
• Current state clinical license preferred.
