Nurse Care Manager - Katie Beckett (Middle-South TN)

Location: Remote within the USA, with travel for face-to-face member meetings and home visits. Ideal candidates live in Marshall, Bedford, Coffee, Maury, Mt Juliet, Murfreesboro, Rutherford, Sparta, Warren, or Williamson.

What you will do

• Manage a caseload of complex pediatric members, including children with disabilities, medically fragile conditions, and behavioral health needs.
• Conduct new-member visits, home visits, ongoing assessments, and evaluations within contractual timelines.
• Develop and maintain person-centered support plans focused on child development, family needs, and community-based services.
• Coordinate care between members, families, providers, private insurance, Medicaid, and internal and external stakeholders.
• Oversee complex cases, utilization management activities, interventions, benefit efficiency, and outcomes.
• Provide digital communication and technical support through online channels when required.

Requirements

• Active Registered Nurse license in Tennessee or an active license in the state of residence if participating in the Nurse Licensure Compact.
• At least 3 years of clinical experience and 5 years of experience in the healthcare industry.
• Pediatric experience, preferably in acute care, involving children with complex medical or behavioral health needs.
• Strong oral and written communication, organization, attention to detail, adaptability, and accountability.
• Basic PC, Microsoft Office, and email skills; successful completion of a digital literacy assessment.
• Ability to travel for member meetings and maintain a valid driver's license, required auto insurance, and an acceptable motor vehicle record when driving for company business.
