Nurse Case Manager

Salary: $105,000–$115,000 per year

What you will do

• Assess applicants by gathering information, reviewing diagnoses, and analyzing medical test results.
• Coordinate drug screenings, medical examinations, and MMPI-2 appointments.
• Update patient medical records after each contact.
• Provide assessment, planning, coordination, evaluation, and monitoring throughout the medical clearance process.
• Coordinate activities among patients, medical providers, and multidisciplinary team members.

Requirements

• Active RN license and a bachelor’s degree in nursing (BSN).
• BSN education must be accredited by an approved nursing accrediting body recognized by the U.S. Department of Education.
• At least 10 years of practical, recognizable clinical nursing experience with an emphasis on case management.
• Strong knowledge of medicine and anatomy, analytical and problem-solving skills, and the ability to manage multiple patients.
• Familiarity with word processors, spreadsheets, electronic medical records, and other software.

Nice to have

• Commission for Case Manager Certification (CCM) or American Case Management (ACM) certification; either may substitute for three years of experience.
