Bilingual Telephonic Care Coach

Location: Remote Illinois; occasional travel to hirify.global offices for training or meetings may be required

Salary: $53,700–$72,600 per year

What you will do

• Evaluate members’ needs and guide members and families toward appropriate care and community resources.
• Coordinate services and resources to support optimal wellness outcomes.
• Identify and resolve barriers that hinder effective care.
• Monitor patient progress through assessments, conversations, data, and active care planning.
• Support members through telephonic coaching and ongoing care coordination.

Requirements

• Bilingual English and Spanish, Polish, Arabic, Creole, or another language.
• Bachelor’s degree in a health and human services field, or an LPN able to be licensed in Illinois.
• At least 2 years of healthcare experience.
• Knowledge of Microsoft Word and Excel and the ability to use multiple electronic information systems.
• Exceptional verbal, written, and interpersonal communication skills.
• A separate room with a locked door is required for a private home office and protection of member PHI/HIPAA information.

Nice to have

• Certified Case Manager certification or a master’s degree in a health services-related field.
• Experience with Medicare and Medicaid recipients.
• Experience with electronic case notes, health promotion, coaching, wellness, case management, discharge planning, or adult acute-care patient education.
• Knowledge of community health, social service agencies, and community resources.
• Residence in Illinois is preferred.
