Case Manager (Care Transition)

Location: Knoxville, Tennessee, USA; candidates must live within 50 miles of Knoxville and travel to area hospitals at least one day per week for in-person visits.

What you will do

• Support members with complex healthcare needs during transitions from hospital care to home and community-based services.
• Conduct face-to-face member visits in area hospitals and collaborate with hospital staff on discharge planning.
• Assess needs, develop care plans, coordinate services, monitor interventions, and evaluate outcomes.
• Serve as a liaison among members, providers, internal care teams, and external customers.
• Support utilization management for complex and non-routine cases and promote efficient use of benefits.
• Connect members with resources, remove barriers to care, and help reduce gaps in care.

Requirements

• Active Registered Nurse license in Tennessee, or an active license in a Nurse Licensure Compact state of residence.
• At least 3 years of clinical experience.
• At least 5 years of experience in the healthcare industry.
• Strong clinical judgment, critical thinking, prioritization, time management, and oral and written communication skills.
• Basic Microsoft Office and email skills, plus the ability to communicate through digital channels.
• Valid driver’s license, compliant auto insurance, and an acceptable motor vehicle record may be required for routine business driving.

Nice to have

• Experience in home health, community-based nursing, public health, health department, or community hospital settings.
• For Select Community roles, at least 2 years of experience in intellectual and developmental disabilities is required.
