LTSS Service Care Manager (Medicaid)

Location: Field-based; applicants must reside in West Palm Beach, Florida. Standard schedule is Monday through Friday, 8:00 AM to 5:00 PM. The role may include home and other site visits.

Salary: $27.02–$48.55 per hour

What you will do

• Evaluate member needs, available resources, and appropriate long-term care outcomes.
• Develop and monitor personalized long-term care and service plans.
• Coordinate services between members, families, caregivers, healthcare providers, specialists, and community resources.
• Educate members and caregivers on service options, referrals, provider instructions, and healthcare benefits.
• Document member information and care management activities in compliance with state, federal, and payer requirements.
• Identify opportunities to improve quality of care and cost-effective service delivery.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or an applicable equivalent combination of education and experience.
• Alternative qualifications include a bachelor’s degree in health, human, social work, or education services with at least one year of qualifying experience, or a high school diploma with three years of qualifying case management experience.
• Experience with LTSS, Medicaid managed care, and HCBS is preferred for success in the role.
• Experience interviewing and assessing member needs, managing caseloads, and applying casework practices.
• Strong problem-solving, communication, organization, time management, computer proficiency, and customer service skills.
• Must reside in West Palm Beach, Florida and be able to perform field-based and occasional home or site visits.
