Care Navigator (Healthcare)

Location: Field-based role; applicants must reside in Montgomery County, Ohio, or one of the surrounding counties: Butler, Warren, Greene, Clinton, Clark, Miami, or Darke County. Work schedule is Monday through Friday, 8:00 AM to 5:00 PM. On-site member visits may be required.

Pay range: $22.94–$38.79 per hour.

What you will do

• Evaluate member needs, barriers to care, available resources, and options for achieving quality outcomes.
• Develop and maintain personalized care plans and identify appropriate providers, specialists, and community resources.
• Coordinate care and services among members, families, caregivers, healthcare providers, and multidisciplinary teams.
• Provide psychosocial, resource, and healthcare-benefit support to members, caregivers, and care managers.
• Monitor care-plan progress and member status, collaborating on revisions or referrals when needs change.
• Document member information and care-management activities in compliance with regulatory and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable knowledge and experience.
• Graduation from an accredited school of nursing is required if holding clinical licensure.
• Current state clinical license is preferred.
• Excellent communication, customer service, problem-solving, organizational, and time-management skills.
• Computer proficiency and the ability to work independently as a self-starter.
• Residence in Montgomery County, Ohio, or an eligible surrounding county is required.
