Field Care Manager, Social Work (LTSS, HCBS, Waiver Programs)

Location: Must reside in Michigan and travel 75%–90% throughout Wayne and/or Macomb Counties. The role is remote with field-based member visits and occasional travel to hirify.global offices for training or meetings. Schedule: Monday through Friday, 8:30 AM–5:00 PM Eastern Time.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ functional, behavioral, environmental, and psychosocial needs.
• Conduct Long-Term Services and Supports assessments and support eligibility determinations.
• Develop or contribute to person-centered care plans and coordinate services across HCBS and waiver programs.
• Identify barriers to effective care and monitor progress toward desired health, safety, independence, and quality-of-life outcomes.
• Collaborate with members, families, caregivers, healthcare providers, interdisciplinary teams, and community organizations.
• Maintain accurate and compliant documentation in accordance with organizational, state, and federal requirements.

Requirements

• Must reside in Michigan and be able to travel throughout Wayne and/or Macomb Counties.
• Active Michigan Social Work license in good standing: LBSW, LLMSW, or LMSW. Limited-license candidates must demonstrate progress toward full licensure.
• At least 2 years of post-degree experience in social work, behavioral health, healthcare, care management, or community-based services.
• At least 1 year of experience with HCBS and/or waiver programs, plus LTSS assessment experience.
• Knowledge of LTSS, HCBS, Medicaid requirements, person-centered planning, member rights, community health, and social service resources.
• Valid state driver’s license, personal vehicle liability insurance, strong communication skills, and proficiency with electronic medical records, care management platforms, Microsoft Word, Outlook, Excel, and web-based applications.

Nice to have

• Fluency in English and Arabic or Chaldean, including the ability to speak, read, and write each language independently.
