CHOICES Care Coordinator (Warren and Cannon Counties)

Location: Remote within Warren or Cannon County, Tennessee, USA. The role includes travel to members’ homes for face-to-face visits and an 8:00am–5:00pm EST schedule with no on-call requirement.

What you will do

• Partner with members and families to identify supports and services that advance health, employment, independence, and community-living goals.
• Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs.
• Conduct objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Apply criteria to authorize appropriate home- and community-based services.
• Confirm that authorized services are being delivered and members’ needs are being met.
• Manage competing requests through critical thinking, prioritization, and effective time management.

Requirements

• Must live in Warren or Cannon County, Tennessee.
• Two years of clinical experience required; three years of clinical-setting experience preferred.
• Active Tennessee registered nurse license, or an active license in the state of residence under the Nurse Licensure Compact Act; alternatively, an active unrestricted LCSW, LMSW, or LAPSW license with a Master’s in Social Work.
• Valid driver’s license; vehicle use requires compliance with applicable insurance and driver-safety requirements.
• Basic Microsoft Office and email skills, strong communication and interpersonal skills, organization, and time management.
• TB skin test, applicable immunizations or medical tests, and a digital literacy assessment may be required.
