CHOICES Care Coordinator- Dickson and Hickman Counties

Location: Remote within Dickson or Hickman County, Tennessee, USA; the role includes travel to members’ homes for face-to-face visits. Schedule: 8:00am–5:00pm EST, with no on-call requirement.

What you will do

• Partner with members and families to identify supports and direct services that promote health, employment, independent living, and community living.
• Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs.
• Conduct objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Apply authorization criteria for home- and community-based services.
• Confirm that authorized services are delivered and members’ needs are being met.
• Manage requests and priorities through critical thinking, problem-solving, time management, and organization.

Requirements

• Must live in Dickson or Hickman County, Tennessee.
• Two to three years of clinical experience, preferably in a clinical setting.
• Active Tennessee registered nurse license, or an active license in a Nurse Licensure Compact state; alternatively, a Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license.
• Valid driver’s license; a TB skin test is required for coordinators working in the field.
• Basic Microsoft Office and email skills, effective time management, and strong oral and written communication.
• Strong interpersonal, organizational, customer service, and digital literacy skills.
