What you will do

• Provide acute, chronic, and preventive care to patients experiencing homelessness, behavioral health challenges, serious mental illness, substance use disorders, complex chronic conditions, or transition from incarceration.
• Assess patients, collect histories, perform physical examinations, order and interpret diagnostics, and prescribe treatment under physician supervision.
• Design and implement care plans supporting patient stability and improved health outcomes.
• Manage patient panels and coordinate care with physicians, nurses, social workers, and outreach staff.
• Use telehealth and AI-driven tools while delivering direct care in field-based settings.
• Document all encounters and escalate atypical or complex cases to the supervising physician.

Requirements

• Active, unrestricted California Physician Assistant license required by the time of hire.
• Successful completion of the Physician Assistant National Certifying Exam, plus National Provider Identifier and DEA numbers.
• Basic Life Support certification.
• Ability to provide direct care outdoors and in dynamic, unpredictable conditions while maintaining independent clinical judgment within scope of practice.
• Ability to travel up to 75% of work time, with a valid California driver’s license and reliable, insured vehicle.
• Strong communication, collaboration, clinical workflow, prioritization, and support-staff coordination skills.

Nice to have

• Bilingual English and Spanish communication skills.
• At least one year of experience with CalAIM populations of focus, including people experiencing homelessness, adults with serious mental illness or substance use disorders, people transitioning from incarceration, and adults with complex medical needs.
• Experience or interest in public health, home care, social work, or outpatient care for vulnerable populations.
