Advanced Practice Provider (NP or PA): Palliative Care (Medtech)

Location: Remote USA

Salary: $120,000–$155,000 annual base salary, plus a performance-based bonus of $10,000–$20,000.

What you will do

• Conduct initial assessments and create palliative plans of care for newly enrolled members.
• Manage a longitudinal panel of high-risk patients through telephone and video encounters.
• Provide advanced symptom management, complex goals-of-care counseling, disease education, prognostication, and support for hospice transitions.
• Evaluate hospital admissions, support transitions of care, identify readmission trends, and help maintain a readmission rate of 15% or less.
• Participate in clinical huddles, interprofessional meetings, panel reviews, and cross-functional care coordination.
• Provide rotating weekend and holiday on-call coverage and support the team during periods of growth or staff absence.

Requirements

• Current, active, unrestricted Nurse Practitioner or Physician Assistant license in at least one U.S. state.
• Willingness to obtain additional U.S. state licenses as required; associated fees are fully covered.
• At least 5 years of advanced practice experience, including at least 3 years in palliative care.
• ACHPN credential, or willingness to obtain it within the first six months.
• Willingness to work across time zones and comfort with telehealth, technology, and a fast-paced environment.
• Strong communication, interpersonal, clinical judgment, and interprofessional collaboration skills.

Nice to have

• ELNEC or EPEC training.
• ICU, oncology, geriatrics, or home-based palliative care experience.
• Interprofessional team leadership or program development experience.
