Registered Nurse Care Manager - Juneau Cty

Location: Remote Wisconsin; coverage area in Juneau County, WI. Travel up to 40% for in-person member visits and occasional travel to hirify.global offices. Monday through Friday, 8:00 am–4:30 pm CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and coordinate with acute and primary care providers.
• Educate members about disease processes, risks, nutrition, exercise, and lifestyle changes.
• Document member information within required DHS contract timeframes and participate in monthly phone contacts.
• Review and remediate member records, assign follow-up tasks in D365, and communicate risks or missing actions to managers.
• Travel to members’ settings for in-person visits and support complex or newly assigned member cases.

Requirements

• Associate degree in nursing.
• Active Registered Nurse license in Wisconsin in good standing.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 coverage.
• Ability to work remotely from a dedicated space protecting member PHI and HIPAA information.
• Ability to travel approximately 40% and work Monday through Friday during CST business hours.
• Completion of required TB screening for this patient-facing role.

Nice to have

• At least one year of care management experience.
• Residence within 45 minutes of the assigned coverage area.
• Previous care management experience.
