RN Care Manager- Eau Claire

Location: Remote Wisconsin, with travel to members' residences within 30–40 miles and occasional travel to hirify.global offices for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' physical, environmental, and psychosocial care needs.
• Guide members and families toward appropriate healthcare, social service, and community resources.
• Develop care plans and monitor progress toward desired health outcomes.
• Identify and resolve barriers that affect effective care delivery.
• Coordinate patient-facing in-home care while working independently within established guidelines.

Requirements

• Registered Nurse with at least 2 years of in-home case or care management experience.
• Experience working with adults and knowledge of community health and social service resources.
• Ability to travel to members' homes within a 30–40 mile range.
• Valid driver's license, personal vehicle, and required automobile liability insurance.
• Ability to work a flexible full-time schedule of at least 40 hours per week.
• Must maintain a private home office with a locked door and high-speed cable or DSL internet; satellite internet is not permitted.

Nice to have

• BSN, case management certification, or motivational interviewing certification.
• 3–5 years of in-home assessment and care coordination experience.
• Experience with health promotion, coaching, wellness, or managed care.
• Bilingual English and Spanish communication skills.
