Care Manager (RN)

Location: Remote in New Hampshire, United States

Salary: $56,200–$101,000 per year

What you will do

• Assess members' physical and behavioral health needs, barriers to care, and social determinants of health.
• Develop and maintain personalized care plans and service plans in collaboration with members, families, caregivers, providers, specialists, and community resources.
• Coordinate timely access to healthcare, social, housing, employment, independent living, justice, and foster care resources.
• Monitor member status, changes in condition, progress toward goals, and revise care plans as needed.
• Provide education on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Document care management activities and support compliance with state, federal, and third-party payer requirements.

Requirements

• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2–4 years of related experience.
• Registered Nurse state licensure and/or compact state licensure.
• Strong experience in both physical and behavioral health is required to support the whole member.
• Must support the New Hampshire Health Families Medicaid population.
